Help! it REALLY hurts when I pee!?

it doesnt hurt in my area, it hurts my lower abs and the area near the rectum, what could this mean and should i be worried? oh yeah the last couple of times that I had to pee, i REALLY had to go. I got the message to my brain that I had to pee and in less than 5 min, i had almost peed myself, im only 19 by the way, HELP
